2026 Recognition in Delivering Value-based Healthcare awarded to Southlake Health

The Canadian College of Health Leaders is proud to announce that Southlake Health’s Southlake@home and Southlake@home PLUS programs have been awarded the 2026 Recognition in Delivering Value-based Healthcare, honouring their leadership in transforming transitional care and redefining how value is created for patients and families.

Designed to ensure individuals receive the right care, in the right place, at the right time, these programs shift recovery from hospital to home while maintaining strong clinical oversight and seamless coordination between hospital teams, community providers, and caregivers. The Southlake@home and Southlake@home PLUS stand as models of value‑based healthcare, reshaping transitional care to put patients and families at the center. By reducing unnecessary hospital days, preventing avoidable emergency visits, and improving patient‑reported outcomes, the programs demonstrate how integrated, patient‑centred design can deliver better health outcomes per dollar spent. Their expansion to support patients with higher complexity, particularly those with behavioural and psychosocial needs, reflects a commitment to inclusivity and ensures that vulnerable populations benefit equally from this innovative model.

The impact of these programs is both measurable and deeply human. Patients report greater confidence managing their health at home, improved communication with care teams, and a stronger sense of safety and trust throughout their recovery. Families consistently highlight reduced stress, clearer care plans, and meaningful involvement in decision‑making. System‑level results reinforce this value: reduced length of stay, a drop in readmissions from 16% to 8%, and a rise in patient satisfaction from 68% to 92%. The project’s system data confirms meaningful reductions in emergency visits and readmissions, demonstrating how the programs generate tangible value for both patients and the health system. By embedding continuous learning, patient‑reported outcomes, digital tools, and collaborative accountability across hospital and community partners, Southlake@home and Southlake@home PLUS offer a replicable blueprint for the future of value‑based healthcare in Canada, one where patients and families are true partners in achieving better outcomes and better experiences.

The College’s Recognition in Delivering Value-based Healthcare aims to increase the profile and understanding of value-based healthcare by honouring an organization, or team, that is deliberate in changing the way that care is delivered, resulting in improved patient outcomes. This means that patients are being optimally cared for at the right time, in the right setting and at the right cost.
